    Case Manager

    Location: Bronx, Brooklyn, Manhattan

    Position: Case Manager

    • Conduct thorough assessments of clients' needs, strengths, and goals.
    • Develop individualized service plans in collaboration with clients.
    • Coordinate access to essential services such as housing, healthcare, and employment assistance.
    • Advocate on behalf of clients to ensure they receive appropriate support from external agencies and community resources.
    • Monitor and evaluate clients' progress regularly.
    • Provide crisis intervention and support during emergencies.
    • Maintain accurate and confidential client records.
    • Collaborate effectively with interdisciplinary teams and community partners.
    • Stay informed about relevant policies, regulations, and best practices.

    Qualifications:

    • Bachelor's degree in social work, psychology, or related field (Master's degree preferred).
    • Previous experience in case management or social services preferred.
    • Strong interpersonal and communication skills.
    • Ability to work independently and as part of a team.
    • Proficiency in Microsoft Office Suite and case management software.
    • Familiarity with resources and services in the Bronx, Brooklyn, and Manhattan areas.
    • Bilingual proficiency (English/Spanish) is a plus.
