Description
Seeking a motivated individual with their Masters of Social Work to join our team.- meeting with veterans upon admission to complete bio-psychosocial, housing plan and identify needs/barriers. Complete intake/discharge paperwork. Assist veterans with housing goals.
Participate in weekly call with VA and abide by Contractual obligations set forth by the Veterans Administration
Must be good at Problem solving/ resourceful and adaptive
- Communicating with outside agencies
- Record cases information, complete accurately all necessary forms
- Provide short term therapeutic intervention to promote stabilization, wellness and positive outcomes among Veterans served.
- Assess and address motivational and psycho
- Adhere to professional standards as outlined by protocols, rules and regulations
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and clientcentered
- Other duties as needed in relation to program efficacy

Pay:
$47, $50,000.00 per year
