    *$1,000 Hiring Bonus, Additional $1,500 Spanish Speaking Bonus*QUALIFICATIONS Education & ExperienceBachelor's degree in a mental health/social service related field ANDAt least 4 years direct service experience in a mental health/social service setting, ORAA Degree and 6 years direct experience in a mental health/social services settingESSENTIAL JOB FUNCTIONSPosition SpecificProvides services to assigned children, youth, and families in the home, office, school, or other community based settings.

    Possesses specific cultural competence to work with and outreach to culturally diverse populations and communitiesProvides services to children and youth involved in the identified systems meeting all County requirements as listed in program scopeProvides Intensive Home Based Services (IHBS) and/or individual rehabilitation to youth as identified in youth's treatment planProvides collateral support to caregivers and significant support persons as identified in youth's treatment planDevelops and facilitates rehabilitative and/or skill building groupsCompletes Functional Assessments and Crisis Plans prior to providing rehabilitation or IHBS as assignedModel and coach positive, constructive attitudes and behaviors in managing difficult situations in school, work, and living situationsCompletes Intake, Core Assessment, Mid Assessment and Discharge packets as assigned.

    Develops and maintains positive relationships with agency and community partnersDevelops and prepares updates to individualized safety plans in coordination with youth's child family team, maximizing the use of natural supports and community based resourcesProvides other Intensive Care Coordination (ICC) and/or case management functions as necessary, securing formal resources, and keeping team members informed of progress and challenges presented.

    Participates in Child Family Team (CFT), multi-disciplinary team, hospital, crisis, psychiatric, court, school, and provider meetings as scheduledProvides and documents all Medi-Cal eligible billable and non-billable services, including but not limited to assessment, plan development, group session, collateral, rehabilitation, In Home Based Services (IHBS) case management, Intensive Care Coordination (ICC) and crisis intervention within required timelinesAttends all staff meetings, consumer meetings, and consultation meetings as scheduledParticipates in the on-call rotationAble to provide or arrange for safe transportation for youth and families to services as neededParticipates in utilization, accreditation, peer review process as assignedWorks collaboratively with all internal programs, departments, and staff to provide support as neededMaintains positive relationships with consumers and customersResponsible for meeting all outcomes as documented in Employee Development PlanCoordinates with program staff to ensure documentation reflects standards that meet or surpass applicable Agency, County, State and Federal standardsAgency SpecificPerforms all duties in a manner consistent with the principles and values of agency, while adhering to applicable professional codes of ethics, the agency's policies and procedures, contractor requirements, and regulatory requirements.

    Model and communicate appropriate positive attitudes toward the agency's Mission, Vision, and ValuesWork collaboratively with all agency programs and staff to provide support as neededUtilize and maintain calendar with all work-related details in order to manage time effectively and share calendar information with coworkersParticipate in on-going training to expand and develop professional skillsPerform other duties as necessary for the agency, as assignedEmployment At-WillEmployment at the Agency is terminable at-will, which means that employment may beterminated at any time, without cause or reason, by either the employee or the Agency.

    Inaddition the Agency may also demote, layoff, transfer or reassign employees at any time at itssole discretion without cause or reason.
