    Case Manager - Registered Nurse Opportunity at Home Health Agency in Indianapolis, Indiana

    An exciting chance for a Registered Nurse looking to transition into Case Management. This Home Health agency is willing to train high-acuity nurses from the emergency room or intensive care unit to become Case Managers. If you're striving to achieve a better life/work balance, then this role is perfect for you. Many staff nurses who have made this transition absolutely love it. This fulfilling role offers the independence to work with and care for patients. With a manageable case load, you can work both in the field and in the office without needing to do charting at home. The position offers Monday to Friday daytime hours with all travel time covered. As a Case Manager, you will conduct full physical assessments, gather medical histories, regularly evaluate patient needs, and involve both the patient and their family in the care planning process. Collaboration with field nursing staff makes this role even more rewarding. Reporting to the RN Administrator, this job offers a great work environment with no on-call requirements.

    Requirements include being a Registered Nurse (RN) with a willingness to train high-acuity nurses. The ideal candidate will have experience in case management and home health care. Candidates must also possess CPR certification and a valid driver's license. Strong communication skills and the ability to engage effectively with medical personnel and patients are essential.

    Compensation for this position includes a competitive salary of 85K plus a company car and tablet. Additionally, there is an excellent benefits package with perks such as 3 weeks of paid time off that can be carried over, paid holidays, 401(k) contributions, and a focus on maintaining a healthy work-life balance.
