    Native American Community Liaison - Albuquerque, United States - University of New Mexico

    Position Summary:

    Act as liaison between UNM Hospitals and the Native American Community. You will help facilitate care according to agreements with Indian Health Service (IHS) and All Indian Pueblo Council (AIPC) alongside other groups. You will receive and resolve complaints, coordinate with hospital leadership, meet with parents and families and provide education to various groups. You will also assist with Care Management and discharge planning. Adherence to Hospitals policies and procedures expected.

      Detailed Responsibilities:
    • Coordinate Native American Health Services patient access programs and related services
    • Work with Pueblos, tribes, and nations to enable access to UNM Hospitals/UNMHSC and extend health resources to rural communities in New Mexico
    • Represent UNM Hospitals at community meetings and promote new programs/policies
    • Prioritize and verify referrals for patient access department
    • Provide leadership for improvement, planning, implementation, and evaluation of programs
    • Serve as a liaison for Native American patients and tribal issues
    • Collaborate with and act as a resource for multidisciplinary teams and community health care agencies for quality care
    • Work to reduce barriers to access to medical service and navigate patients through community resources and financial assistance programs
    • Collaborate with medical providers, multidisciplinary team members, and community resource providers to help patients meet treatment goals
    • Coordinate with community and social workers to provide consultative services to Clinical Social Work and Discharge Planners and assist with admissions for Native Americans
    • Use communication strategies for individuals and groups to achieve outcomes and patient satisfaction
    • Build and maintain relationships with community agencies and represent UNM Hospitals at community events
    • Conduct educational sessions and presentations on the Native American community and their needs
    • Facilitate and participate in conferences to evaluate interdisciplinary dynamics and goals
    • Work with healthcare teams for continuous care and promote communication between patients, family, healthcare team members, and significant others

    Qualifications:

    • Associate Degree (Bachelor's degree preferred) in a related discipline
    • 2 years of directly related experience (3 years and fluency in English, Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo preferred)
    • Credentials not applicable/not required
    • Physical conditions include light work
    • No or minimal hazard and physical risk
    • May require travel to various work sites and may work rotating shifts, holidays, and weekends

    Related education and experience may substitute for one another on a year-for-year basis.
