    DUTIES & RESPONSIBILITIES:

    • Understand and uphold SEPs Mission, Vision, and Values.
    • Comply with all applicable laws and regulations.
    • Work with manager to interview and evaluate candidates for open positions.
    • Consult with manager to provide coaching and document performance concerns immediately, thoroughly, and accurately.
    • Ensure new employees are properly oriented and are aware of the performance expectations of their position. Team Leader will be new associates PAL during training.
    • Accurately maintain and approve associate timecards via payroll system by the deadlines set by payroll.
    • Analyze staffing levels and schedule associates to maximize efficiency and productivity. Maintain staffing levels based on established company metrics.
    • Coach associates on meeting established work goals based on various clinical and other key measurements.
    • Evaluate associate performance and provide feedback in a timely manner. Provide feedback & examples for annual performance evaluations for direct reports, obtaining stakeholder feedback as necessary and appropriate.
    • Support management in maximizing retention and minimizing associate turnover. Recognize employees for outstanding performance and work to keep associates highly engaged.
    • Maintain a motivational, positive, and supportive working environment for associates to include praise and recognition.
    • Communicate and administer policies and procedures as outlined. Consult with manager as appropriate to address policy and procedure issues.
    • Support management in resolving associate and provider issues.
    • Communicate goals, objectives, and other defined performance objectives to manager.
    • Conduct regularly scheduled quarterly meetings with associates to review department performance goals and objectives. Prepare updates to manager in a timely manner.
    • Other duties and responsibilities as assigned are completed on a timely, thorough, and accurate basis. This includes the potential to fill in for other roles during times of short staffing.
    • Run discharge reports to ensure direct reports are up to date on assignments. Evaluate and distribute workload to resolve reports that are behind.
    • Collaborate with manager and provide team updates bi-weekly at minimum.
    • Facilitates effective communication, coordinates services, addresses barriers, and provides education and guidance for patients related to current health concerns.
    • Collaborates with community resources (e.g. home health, Durable Medical Equipment, support groups) to ensure smooth handoffs and/or orders from the Ambulatory Care Management team.
    • Completion of RN Care Coordinator role duties such as Medication Reconciliation and Chronic Condition Education.

    EDUCATION:

    Minimum:
    Bachelors Degree and active RN Licensure


    YEARS OF EXPERIENCE:

    Minimum: 3+ years experience as a nurse.


    Desired: 1+ years supervisor/coaching experience


    LICENSES AND CERTIFICATIONS:

    Minimum:
    Active RN Licensure; Must have reliable transportation for local travel between work locations.
