We are seeking a highly skilled and compassionate *Registered Nurse* to join our team at a long-term care facility in Bayside, Queens.
In this role, you will be responsible for providing high-quality nursing care to patients.You will work closely with patients, their families, and the healthcare team to develop and implement care plans, monitor patients' condition, and administer medications.
*Duties and Responsibilities:1)
Resident Assessment:
As a Registered Nurse you will perform comprehensive assessments of residents' health conditions, including physical, cognitive, and emotional status.
Document findings and develop individualized care plans.2)
Care Planning and Implementation:
Create care plans based on residents' needs, incorporating nursing interventions, medication administration, wound care, and other necessary treatments. Implement and monitor care plans, ensuring residents' comfort, safety, and well-being.

Medication Management:
Administer medications as prescribed by physicians, ensuring accurate dosages and monitoring for adverse reactions. Maintain medication records and collaborate with pharmacists to manage medication supplies.
4)
Collaboration and Communication:
Work collaboratively with the interdisciplinary team, including physicians, therapists, social workers, and caregivers, to coordinate and integrate resident care.
Maintain clear and accurate communication regarding residents' conditions, progress, and changes in care plans.5)
Monitoring and Evaluation:
Regularly monitor residents' health status, vital signs, and response to treatment. Evaluate the effectiveness of care plans and make necessary adjustments based on changes in residents' conditions.
6) Patient
Education:
Educate residents and their families about their health conditions, treatments, and self-care techniques. Provide guidance on lifestyle modifications, medication management, and preventive measures.
7)
Emergency Response:
Respond to medical emergencies and provide immediate nursing interventions. Collaborate with the emergency response team to stabilize residents and ensure their safety.
8)
Documentation and Reporting:
Maintain accurate and up-to-date medical records, including nursing assessments, care plans, progress notes, and incident reports. Adhere to facility policies and legal regulations regarding documentation and reporting.
9)
Infection Control:
Implement infection prevention and control measures to minimize the risk of healthcare-associated infections. Follow established protocols for hand hygiene, personal protective equipment (PPE) use, and isolation precautions.
