- Participates in resident and family education, as well as discharge planning
- NYS license as a registered nurse
- Graduate of accredited school of nursing
- At least 6 months prior experience in a long term care or short term rehabilitation setting strongly preferred
- Some hospital in med-surg department experience preferred
