TITLE
:
COMMUNITY OUTREACH COORDINATOR

REPORTS TO:
VP of Community Relations


STATUS:
Non-Exempt


RESPONSIBILITIES:


  • Cultivate business relationships with the community and facilities within our service areas for increasing patient referrals.
  • Actively recruits participants through strategic and effective community outreach, needs assessment and community events.
  • Board and Care coordination of benefits and the ability to identify board and cares within our service area.
  • Obtain connections with facilities to maintain referral sources.
  • Act as a liaison with facilities
  • Other duties as assigned.

Now with 5 clinics, approximately 50 providers, and 3 mobile clinics, ROADS is continuing its mission of expanding access to quality health care for underserved communities.


Pay:
From $22.00 per hour

Expected hours: 40 per week
