- Collaborate with the Patient Coordinator to ensure accurate clinical documentation for case openings.
- Initiate and update the plan of care, medical orders, Medication Administration Records (MAR), and Treatment Authorization Requests (TAR) as required.
- Conduct monthly client visits both in-home and virtually as needed.
- Provide in-home supervision every 60 days or as deemed necessary.
- Act as a liaison between Summit and the community, clients, facilities, etc.
- Evaluate the outcome of care and provide assistance to guarantee services meet the client's requirements.
- Inform physicians and other staff of changes in the client's needs.
- Active RN license in the state.
- 2+ years of supervisory experience.
- Prior Home Care experience.
