    Housing Works was founded in 1990; With a long-term commitment to AIDS advocacy. Housing Works established New York State's first harm reduction-based, OASAS-licensed outpatient drug treatment program.     Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

    Benefits:
    We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit.     This position is included in a bargaining unit of Housing Works' New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union ("RWDSU"). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a "CBA") between Housing Works and the RWDSU

    Compensation Range: $20/hr - $22/hr commensurate with experience

    Overview:

    The Community Health Worker provides direct supportive services to all program-enrolled clients with peer-to-peer assistance with and accompaniment to medical appointments and social services. They assist with case-finding and re-engaging clients into care and support the overall service delivery functions performed by the program's social workers.

    Responsibilities:
    • Collaborate on a treatment team for clients enrolled in mental health and substance use counseling services. Identify substance use history, triggers, and past treatment history, and coordinate services with the treatment team.
    • Assist in the program's efforts to improve treatment adherence and viral load suppression.
    • Assist with outreach and recruitment of new clients at health fairs, community forums, and other activities.
    • Provide wellness support services to individuals, families, and groups, biomedical counseling to clients and partners, provide client assistance with and accompaniments to appointments, and outreach for engagement and reengagement.
    • Assist in securing benefits and entitlements, and help with obtaining services to support independent living, self-care, nutrition, and money management.
    • Document services in progress notes in a timely manner, gather and contribute data in reports to funders, and utilize electronic medical record systems.
    • Participate in funder-required training to develop knowledge of working with people who use substances and people living with HIV/AIDS.
    • Follow agency policy and procedures.
    • Ability to work a hybrid schedule, 3 days onsite and 2 days remote.
    • Perform other responsibilities and duties as assigned.
    Minimum Requirements
    • High school diploma or equivalent.
    • May have been trained by an organization that offers peer education services.
    • Preferred if eligible to work toward New York State AIDS Institute Peer Certification and willing to complete certification within the first year of employment.
    • Must have some experience working with individuals living with HIV/AIDS.
    • Ability to communicate orally and in writing.
    • Bilingual (English and Spanish) preferred.
