    RESPONSIBILITIESOversee the overall management and coordination of the case management team which serves individuals experiencing homelessness. Build, develop and scale new and existing programs from the ground up. Work with community members and other stakeholders to identify necessary programs and services. Oversee administrative aspects of programs to meet the objectives of the stakeholders. Analyze data to determine the effectiveness of the program. Suggest and implement improvements to programs and services. Serve as the central point of contact for the program, ensuring that all program participants obtain and maintain the desired supportive care. Serve as the liaison with the local government health agencies, managed care providers and other community-based organizations and partners. Ensure program compliance and effective operations. Facilitate weekly team meetings and case conferencing utilizing a trauma-informed care lens and harm reduction approach. Work closely with mental & physical health providers, supportive services agencies, and property management agencies to ensure that client needs are being met, and to develop creative interventions focused on improving the safety and well-being of our most marginalized clients. Monitor and ensure documentation requirements are being met which includes but is not limited to; timely enrollment of program participants, initial, quarterly, and annual participant assessments; weekly reporting; the creation and implementation of individual service plans; client budgeting; reporting; etc. In collaboration with the Leadership team, monitor all administrative aspects of the organization's contract, including billing and contract maximization in compliance with contract goals and outcomes in a timely manner. Ensure proper linkages to community resources by case management and ensure the usage of the "Whatever it takes" approach to case management.

    QUALIFICATIONS
    • Have the following educational credentials: Bachelors with a minimum of three years of leadership experience - Have a minimum of two years of healthcare administration, experience working with managed care providers
    • Have a strong foundational knowledge of chemical dependency and substance abuse treatment
    • Are passionate about leadership – you'll be providing direct supervision and management of assigned staff
    • Are super detail-oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.
    • Are critical thinkers, and enjoy problem-solving while being a team player.
    • Can work in a high tolerance harm reduction model with clients who have multiple barriers.
    • Learn quickly and work effectively with a wide range of constituencies and minimal supervision.
    • I would describe myself as a self-starter with the ability to stay ahead of the curve and thrive in a fast-paced work environment.
    • Have a valid Driver's License with an acceptable driving record, and have a car (less than 25% travel)
    • Can type at least 30 WPM, and can require, sit, bend and carry/lift up to 35 pounds
