_Anchor Health Home Care is seeking a dynamic and compassionate individual to join our team as a Case Manager - Patient Support Specialist.

As the face of Anchor for our patients in our HCBS Department, you will play a pivotal role in providing exceptional support to our patients and their families.

_

  • Responsibilities:_
- _Communicate effectively with patients and their families to address any concerns or difficulties they may encounter on a daily basis._
- _Provide superior customer service and support in a compassionate manner._
- _Utilize strong organizational skills to prioritize and handle multiple tasks efficiently._
- _Manage projects by establishing objectives, milestones, and creating necessary documentation._
- _50% travel required _
- _Work autonomously while also collaborating effectively with the team._
- _Maintain professionalism and a positive attitude in high-pressure environments._

  • Qualifications and Experience:_
- _Bachelor's degree is required for this role._
- _Minimum 1 year of customer service experience._
- _Proficiency with computers, including Microsoft Word and Excel._
- _Ability to learn and operate scheduling and training software systems._
- _Highly detail-oriented with excellent time management skills._
- _Healthcare experience preferred; homecare experience is a plus._
- _Bilingual in English and Spanish preferred._
