    Community Based Therapist - San Francisco, United States - Seneca Family of Agencies

    Description

    The Community Based Therapist coordinates services and planning for children who are referred through social service, probation or mental health departments and are in the process of stepping down from institutional into family or family-like settings. Services will be provided almost exclusively in the community in families' homes, group homes, foster homes, schools, etc. The primary role is to ensure that all parties are working together toward stability for the child and family. Clinicians engage clients in meaningful and authentic relationships while providing a safe, predictable, and positive environment that aligns with our clients' values and cultures.

    • Promptly meet with clients and caregivers on a regular basis to further treatment goals utilizing culturally informed and best practice modalities.
    • Coordinate with multi-disciplinary teams for the purpose of care planning and support.
    • Collaborate with agency staff, caregivers, other professionals and interested parties to formulate and implement individual client care plans, behavioral programs, and interventions.
    • Cultivate and maintain relationships with community members and providers, as needed.
    • Complete assessment and evaluation process of clients in order to develop and implement the care plan.
    • Provide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriate.
    • Chart mental health notes, care plans, outcome measures, incident reports, and other documentation accurately and in a timely manner.
    • Maintain availability for crisis intervention services, such as on-call rotation, if applicable.
    • Participate in individual and/or group supervision.
    • Comply with all child abuse reporting and Medicaid/Medi-Cal requirements in accordance with standards set by Seneca.
    • Commit to and engage in self-evaluation as well as organizational goals, strategies, and systemic practices regarding diversity, equity and inclusion.
    • Utilize crisis communication and de-escalation techniques as per Seneca's EQ-4 model training. This may include intervening with the clients' behavioral program through physical management of the clients, if applicable. Training in additional crisis intervention modalities such as CPI may be required.
    • All other duties as assigned by program, region, and agency leadership.

    Required:

    • Master's degree in social work/psychology/counseling.
    • Current licensure or eligibility for licensure with the applicable regulatory agency responsible for licensing, examination, and enforcement of competent mental health practice standards.
    • Ability to work as part of a multidisciplinary team.
    • Ability to work with a diverse population.
    • Good organizational and writing skills.
    • At least 21 years of age.
    • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements.
    • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies' insurance policy.
    • Proof of up-to-date COVID-19 vaccination or an HR-approved medical/religious exemption.

    Preferred:

    • Experience working with children in out-of-home care
    • Full-time
      • Mon - Fri
      • Flexibility schedule to work some evenings and weekends
      • Hybrid; provide in-person services & complete documentation remotely
    • Starting at $72,100 - $86,100 per year, actual salary dependent on creditable experience above the minimum qualification
    • Additional $4000 salary increase for BBS licensure
    • Mileage reimbursement
    • $2,000 sign-on bonus
