- Offer care coordination services for effective management of clients' health needs.
- Ensure accurate documentation to meet regulatory standards.
- Demonstrate understanding of admission and discharge criteria.

- Respond to inquiries and referrals promptly.
- Establish therapeutic relationships with clients for best treatment outcomes.
- Administer medications as necessary and follow physician's orders.
- Ensure safety in the environment through vigilant monitoring.
- Follow infection control policies and serve as a consultant on precautions.
- Monitor lab work and consult medical providers when needed.
- Screen clients for health issues and provide education on managing medications.
- Document client's progress and communicate effectively with team members.
- Provide supervision and structure within the treatment milieu.
- Attend meetings and complete documentation accurately and timely.
- Adhere to network policies and procedures, including confidentiality.
- Participate in required training programs.
- Perform additional duties within the scope of the position.
- Registered Nurse with current Colorado license
- BSN preferred
- Minimum of 2 years of clinical nursing practice experience.
- 1 year in patient behavioral health experience preferred.
- Demonstrated ability in nursing assessment and interpretation.
- Experience in care management and discharge planning.
- Strong communication and problem-solving skills.
- Interpersonal skills and ability to work independently and collaboratively.
- Proficiency in PC-based software programs.
