    Description

    Job Summary:

    Provide nursing care to patients under the supervision of a Registered Nurse, using observational skills to evaluate nursing interventions and support medical staff in delivering optimal patient care within established guidelines.

    Key Responsibilities:

    • Adhering to Kaiser Permanente's Policies and Procedures, as well as state, federal, and local regulations.
    • Demonstrating exceptional customer service to patients, visitors, physicians, and colleagues by displaying courtesy, empathy, and respect.
    • Delivering therapeutic patient care based on physician and RN directives within the scope of practice.
    • Transcribing physicians' orders following established protocols.
    • Collaborating with registered nurses to adjust care plans and contribute to the development of nursing care plans.
    • Administering specified medications through various methods.
    • Performing treatments such as catheterizations, suctioning procedures, and enemas; providing care for isolated patients.
    • Conducting patient care activities including vital sign measurements, wound care, and specimen collection.
    • Assisting medical staff in examinations and treatments, as well as identifying and responding to changes in patients' conditions.
    • Identifying barriers to learning and providing patient education as directed by the RN.
    • Documenting patient care details accurately in medical records.
    • Maintaining and ensuring the proper functioning of patient care equipment.
    • Engaging in effective verbal and non-verbal communication.
    • Participating in meetings and ongoing training sessions.
    • Supporting in the procurement and distribution of unit supplies and equipment.
    • Accepting and implementing orders from physicians.
    • Providing various medical treatments and documenting care provided.
    • Assisting in discharge coordination processes.
    • Required to travel as part of home visits to patients.
    • Promoting and upholding exceptional customer service standards.

    Minimum Qualifications:

    Experience

    • At least one (1) year of recent experience as an LVN in a home health/hospice or acute care environment within the last three (3) years.
    License, Certification, Registration
    • Driver's License (California)
    • Vocational Nurse License (California)
    • Basic Life Support certification
    • I.V. Certification

    Additional Requirements:

    • Driving/traveling for home visits.

    Notes:

    • Required to work weekends on a rotating basis.
