Position Summary:

Crossroads Community Outpatient Management Team (CCOM) is a Crossroads Outpatient program composed by a multidisciplinary team that provides direct, patient centered care to members who are General Mental Health with a SUD, and are in need of integrated care services with the goal of assisting vulnerable adults to gain stabilization, increase their ability to function independently and participate in meaningful activities in the community.

The Crossroads Community Outpatient Management program targets high needs, high risk adults in order to improve discharge planning from the residential services, and continue their engagement in the system of care post residential treatment.


Essential Duties and Responsibilities:


  • Coordinating screenings and client admissions as well as ensuring that the member is provided with services in direct relation to their integrated service plan.
  • Direct care services such as referrals for ancillary services.
  • Participation in staffing's and adult recovery teams, benefits counseling, identification and engagement in activities congruent with a sober lifestyle.
  • Coaching and support in independent living skills, and all activities that will enable members to navigate the system of care more independently, and enhance their quality of living and daily functioning.
  • Assisting members by increasing employment supports, reducing hospitalizations, reducing recidivism, increasing daily functioning as well as assisting with ongoing mental health and physical stabilization.
  • Collaborate to reduce homelessness, ensure members have a safe and stable living environment as well as assisting with any discharge planning.
  • The Care Specialist does not have a fixed location, the position will travel to various residential Crossroads facilities as direct by Supervisor.
  • Perform other duties as necessary.

Qualifications:


  • Valid AZ driver's license
  • 1 year experience providing direct patient care and working in multiple settings related to substance abuse and mental health
  • Peer Recovery Support Certification
  • Must currently have a minimum of 6 month of continuous sobriety.
  • Current CPR/First Aid training
  • High School Diploma or GED
  • Preferred 1 year or more of experience working in a mobile outpatient case management team

  • While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.
