- The Geriatric Nursing Program Manager will oversee and lead all aspects of the Hospital of the University of Pennsylvania's (HUP) American Geriatrics Society CoCare Hospital Elder Life Program (AGS CoCare:HELP). AGS CoCare:HELP is a comprehensive program designed to help prevent delirium and functional decline in hospitalized older adults. The HELP interventions are delivered by nursing students in a role titled Student Geriatric Associates (SGAs). The SGAs report to the Geriatric Nursing Program Manager who manages all aspects of their role and responsibilities. The Geriatric Nursing Program Manager will also oversee the work of SGA Coordinators - clinical nurses in a rotating part-time role - who support the SGAs in the practice environment and manage some aspects of SGA operations (e.g., scheduling, competency validation).The Geriatric Nursing Program Manager works collaboratively with the Supporting Older Adults at Risk (SOAR) team, which primarily includes Geriatric Nurse Practitioners as well as other interdisciplinary team members.
- The Geriatric Nursing Program Manager will also engage in activities related to implementation of the Age-Friendly Health Systems (AFHS) 4Ms Framework. Delivering care based on the 4Ms Framework - what matters, medications, mentation and mobility - helps reduce readmissions, decrease length of stay and improve health outcomes for older adults. The Geriatric Nursing Program Manager will also coordinate activities related to HUP Nursing's membership in NICHE (Nurses Improving Care of Healthsystem Elders)-a set of tools and resources that support nurses in optimizing care of older adults. These aspects of the role position the Geriatric Nursing Program Manager as a leader and a resource to others in the Department of Nursing as well as other interprofessional team members.
- Direct and supervise personnel; oversee interviewing, hiring, training, and ongoing evaluation of staff performance. Support team building and conflict resolution.
- Assume 24 hour accountability for program operations, inclusion provision of leadership to improve patient care and support staff professional development to achieve nursing goals.
- Plan, organize and coordinate activities related to quality improvement of the program. Monitor adherence to protocols, and actively participate in the evaluation of the program's outcomes in relation to established goals and implement changes to improve efficiency and effectiveness.
- Oversee and implement program operations, including program meetings, maintenance of records and materials, program communications, and key information exchanges with nursing leaders and hospital administration.
- Direct the implementation of long-range strategic goals and program development plans. Continually reevaluate program interventions and outcomes in alignment with changing staff and patient needs in a dynamic healthcare delivery landscape.
- Act as a liaison for care coordination with the interprofessional team. Provide education and secure resources to support improvement of hospital-wide care of older adults.
