    Job Overview

    • Company Mission: Providing seniors the opportunity to age gracefully in their own homes and communities.
    • Program: PACE (All-Inclusive Care for the Elderly) offering comprehensive medical services.
    • Team Structure: Collaborative Interdisciplinary Team (IDT).
    Position Details
    • Role: Medical Director
    • Reporting to: Chief Clinical Officer
    • Team Role:
    • Central member of the IDT.
    • Prioritizes participant well-being.
    • Ample time for personalized care.
    Key Responsibilities
    • Comprehensive Assessments & Care Plans:
    • Perform comprehensive assessments for participants.
    • Develop individualized care plans with the IDT.
    • Participant Changes & Care Coordination:
    • Communicate participant changes.
    • Coordinate care planning and ensure 24-hour care delivery.
    • Collaborate with network providers and other IDT members.
    • Directing Nurse Practitioners:
    • Oversee participant use of medical specialists and inpatient care.
    • Participate in rotating night and weekend call schedules.
    • Nursing Home Visits:
    • Conduct regular visits to participants in nursing homes.
    • Follow PACE standards and participant needs.
    • Outcome & Utilization Review:
    • Review outcome and utilization data with the IDT.
    • Identify opportunities for improved care and operations.
    Qualifications
    • Education & Certification:
    • Completed a residency program accredited by the Accreditation Council for Graduate Medical Education.
    • Master's or doctorate degree.
    • Board-certified in Internal Medicine or Family Practice.
    • Board certification in Geriatrics (preferred).
    • Active California Physician License (without restrictions).
    • State CDS Certification and DEA Registration.
    • Skills & Expertise:
    • Excellent clinical, organizational, and communication skills.
    • Experience working with frail, elderly participants and their families.
    • Deep understanding of chronic care and geriatric issues.
    • Knowledge of best practices in geriatric care.
