    ABOUT

    My company is called Living Solutions Plus, PLLC ), and we are a mobile outpatient therapy company. We go to the homes of seniors and provide traditional Occupational Therapy, home assessments/modifications, and LSVT BIG. This is a relatively new and small company, so as of right now the position is PRN, however with the opportunity to grow into larger caseload as we add clients.

    I am looking for an OT to complete initial evaluations for patients inside their home. Bonus points if you are certified in LSVT BIG, or have additional geriatric-related certifications. If you are LSVT BIG certified, you will have the opportunity for additional caseload for LSVT BIG treatments.     PURPOSE

    To provide the highest level of client care in compliance with AOTA practice guidelines and scope of practice in clients homes in accordance with physician's orders; to ensure client satisfaction, and compliance with the AOTA Code of Ethics and Standards of Practice.

    QUALIFICATIONS

    • Graduate of an accredited, approved Occupational Therapy program.
    • Licensed Occupational Therapist in MI. Must comply with all MI requirements for continuing education.
    • Certified Occupational Therapist Registered (OTR) by the National Board in Certified Occupational Therapy (NBCOT)
    • Valid National Provider Identifier number (NPI), or willing to obtain
    • Credentialed with Medicare in the State of MI, or willing to become credentialed
    • Must understand Medicare and other provider guidelines and compliance, or be willing to learn
    • Must know and follow the AOTA Code of Ethics and Standard of Practice.
    • Must have experience working with clients in their home, or be comfortable with client homes.
    • Must have experience working with older adults.
    • CPR certified.
    • Strong organization and communication skills.
    • Must be reliable and flexible to meet the needs of the clients.
    • Must have reliable transportation and be comfortable with driving to client homes.

    ESSENTIAL JOB FUNCTIONS

    • Provides outstanding client care and appropriate treatment following established clinical procedures and techniques including:
    • Initial evaluations and establishes appropriate long- and short-term measurable goals and plan-of-care.
    • Performs appropriate and standardized assessments according to the client's diagnosis and develop a plan of care accordingly.
    • Follows through with the established plan of care for each client.
    • Provides appropriate occupation based, client centered treatment following established procedures and techniques, and utilizes best practice and evidence-based practice.
    • Maintains the highest level of client confidentiality.
    • Provides documentation for all evaluations, treatments, home programs, re-evaluations, re-assessments and discharges in compliance with regulatory, licensing, payer, accrediting and company guidelines.
    • Maintains exceptional client, healthcare provider and staff relationships.
    • Demonstrates excellent time management skills with the ability to perform multiple tasks simultaneously.
    • Coordinates scheduling of clients for evaluations and treatment in coordination with clients and other staff.
    • Practices OT consistent with the AOTA Code of Ethics and Standards of Practice.
    • Supervises any COTA in accordance with industry standards.
    • Maintains professional license, and malpractice/general liability insurance and provides current copy to owner each year.
    • Maintains up to date techniques and trends in occupational therapy.
    • Reports all client related issues to owner.
    • Ensures that all equipment is maintained and in proper working condition to ensure client safety.
    • Assists in tracking client information – return to Dr. dates, benefit information.
    • Maintains compliance under:
    • HIPAA
    • OSHA
    • Medicare and other insurances
    • State and federal regulations
    • Any other
    • Attends and participate in all staff meetings and trainings.
    • Assists in the marketing of practice.
    • Works with the owner to ensure quality assurance and client satisfaction.
    • Meets with the team members as necessary to discuss client care and other clinical issues
    • Specific tasks and other related duties as requested and/or assigned by the Owner.
    • Phones are to be kept away from clients as texting, calling and internet surfing are not permitted during client care and when driving.
    • Notice of termination of employment and requests for any time off must be given a four-week advanced notification.
