    Summary of the Position:


    The Director, Healthcare-Community Partnerships (HCP) is responsible for leading and directing the Healthcare-Community Partnerships team, which supports PHS' growing work to build reliable, trustworthy pathways between healthcare and community-based organizations that improve the social drivers of health.

    Since 2018, PHS has developed value propositions for healthcare and community partners leading to the design and implementation of initiatives to improve food security, promote vaccine equity and facilitate benefits access for thousands of New Yorkers, to name a few.

    During this time, PHS also developed and now manages WholeYouNYC (WYNYC), a coordinated and accountable community resource network, which connects over 13,000 New Yorkers annually to much needed community resources.

    The Director will apply effective management strategies, tools and processes to oversee the various healthcare-community partnership teams delivering high-impact projects and network services critical to improving health equity and access in New York City.

    The Director will coordinate and oversee all aspects of these partnerships including, but not limited to, proposal development, contracting, onboarding, monitoring, close-out and evaluation.

    The Director will nurture critical relationships with key government, healthcare and community stakeholders to ensure PHS continually delivers value to our stakeholders.

    The Director will also support strategic business development by crafting value propositions and providing subject matter advisory input into pitch meetings, HCP / WYNYC marketing materials and planning discussions that enable PHS to grow our healthcare-community partnerships and Neighborhood Health footprint.

    The Director is a member of our PHS Senior Management Team and Neighborhood Health Leadership Team.

    Specifically, the Director, Healthcare-Community Partnerships, work closely with the Senior Director, Healthcare Innovation, other PHS management and staff to:


    • Oversee the development and implementation of PHS' healthcare-community partnership ensuring professionalism and compliance with PHS and funder policies and contractual commitments.
    • Drive healthcare community partnerships forward using effective, project management techniques that enable reliable tracking and monitoring of progress over time.
    • Serve as a liaison to and nurture positive relationships with funders and other stakeholders across the government, healthcare and social service sectors.
    • Actively support development efforts to increase and diversify funding for innovative, collaborative and sustainable healthcare-community partnerships that advance health equity.
    • Develop proposals and scopes of work for grants and other funded opportunities that expand PHS' healthcare-community partnerships.
    • Recruit, mobilize and manage a high-performing team of professionals responsible for carrying out the activities associated with our healthcare-community partnerships.
    • Develop and effectively manage budgets for healthcare-community partnership projects including staff allocations, preparing and submitting budget modification requests and ensuring high-quality, on-time deliverables.
    • Provide effective people management, coaching and leadership to healthcare-community partnerships staff to achieve business outcomes and foster an environment of career development and growth.
    • Work collaboratively with Quality & Evaluation to continuously evaluate the reach, outcomes, impact and value of our healthcare-community partnerships and their contribution to health equity.
    • Collaborate closely and effectively with PHS' Senior Leadership Team, Neighborhood Health Leadership Team and other PHS teams to develop creative strategies and implement bold solutions in alignment with PHS' overall strategy.
    • Provide programmatic subject matter expertise to support the enhancement of technologies (e.g., referral platform) used by PHS' healthcare-community partnerships and WYNYC network services.
    • Represent PHS' healthcare-community partnerships at conferences, symposia and community events.
    • Stay up to date on public health and health equity-related trends, threats and emerging opportunities.
    • Other duties as assigned and prioritized by the Senior Director, Healthcare Innovation.

    Qualification and Requirements:

    • Bachelor's degree required. MPH, MPA or related degree, preferred.
    • Minimum of 5 years' experience in project management, preferably in a public health or not-for-profit setting with responsibility for program development, implementation, management and evaluation.
    • Foundational knowledge of the healthcare and social service sectors to include a solid understanding of the network of health and human service providers in NYC / NYS and related government agencies.
    • Excellent verbal and written communication skills, including grant proposal writing.
    • Skilled at people management and integrating practices to foster collaborative, high-performing teams.
    • Organized, detail-oriented and able to work independently.
    • Dedicated to helping improve the lives of vulnerable and under resourced communities.
    • Salary: $90, $90,000.00
