    Description
    Job Responsibilities:
    • Provides for safe and effective use of medications through comprehensive medication therapy management, consulting with members of the healthcare team and obtaining prior authorizations.
    • Communicates with patients, clinicians and medical providers in a clear and concise manner.
    • Prevents, detects, and resolves medication-related problems and makes appropriate, patient-centered medication therapy recommendations.
    • Manages medications per collaborative practice agreement.
    • Counsels patients and caregivers on medication use and disease management, safe administration measures, and side effect management strategies.
    • Provides clinical consultation and clarification to practitioners such as appropriate, cost-effective therapeutic alternatives to clinical and medical staff as needed.
    • Teaches, mentors and precepts pharmacy students and residents
    • Disease state management, including but not limited to: heart failure, hypertension, diabetes, atrial fibrillation, pain management, lipid management, antimicrobial stewardship, and mental health
    • Medication reconciliation: including objective and subjective data collection, assessment of discrepancies, prioritizing obstacles, determining resolutions, and documenting efficiently
    • Works with healthcare team to maximize department ratings including Star measures, PCMH and MTM.
    • Performs other duties as assigned.
    EDUCATION: Must hold current Pharmacist's License and have or obtain licensure in the state of West Virginia.

    EXPERIENCE: Residency or Board of Pharmacy Specialties (BPS) Certification preferred or must be obtained within 3 years of hire; Minimum 2 years' experience in clinical patient care management preferred.
