    Case Manager Supervisor - Manhattan, United States - Children's Rescue Fund

    The Case Manager Supervisor is responsible for overseeing a team of Case Managers to ensure the support and resources necessary to promote self-sufficiency and independent living skills for families' w/children residing in temporary shelter are being provided in accordance with CRF, DHS and OTDA standards.

    *PRIMARY RESPONSIBILITIES:

    • Oversee and train the casework staff to follow DHS Client Assistance and Rehousing System (CARES) System regarding Intake, Shelter Assessment, Service Plans/Independent Living Plans, Provider Housing, Transfers/Next Step Transfers, Shelter Discontinuance, Incident Reports, Facility Bed Management, etc.
    • Provide advocacy and referral services to ensure clients receive appropriate benefits, entitlements and services thereby enhancing their transition to permanent housing while minimizing the length of time families are homeless.
    • Coordinate services to stabilize the family unit as needed to deal with problems of substance abuse, improve parenting and / or housekeeping skills, and employment / money management skills so the goal of permanent housing may be realized.
    • Monitor and provide guidance to CW staff as it relates to special needs cases such as Mental Health, Weekly ACS and Crib, and Substance Abuse.
    • Ensure that CW staff is completing follow up with Children Services, completing weekly ACS and Crib inspection in addition documenting all communication and findings.
    • Utilizing the DHS CARES System Supervisory Function monitor teams' performance.

    Assess and identify the clients' needs, formulate appropriate plans of service, monitor and document progress of service plans and document all significant interactions with or on behalf of the clients to ensure the clear, effective delivery of service to families and the provision of high-quality social services that are following the State, City and Agency regulations.


    • Develop and maintain appropriate helping relationships with the families to assist them in making the most of their residency at the facility and facilitate their active involvement in the achievement of service goals.
    • Provide appropriate individual counseling and crisis intervention services to families to help stabilize and minimize the negative impact of the family's displacement.
    • Maintain familiarity with off-site and on-site program resources for homeless families and refer families to appropriate resources to ensure that they receive the most beneficial services.
    • Facilitate groups, as designated, to address family issues, enhance life skills, and assist families in achieving self-sufficiency and a successful transition to permanent housing.
    • Coordinate client services provided by other social services staff to ensure that families receive the necessary and appropriate services in a timely fashion and to ensure compliance with State and City regulations.
    • Facilitate effective communication among staff, both within the Casework Unit and with other staff members. Ensure the timely and accurate transfer of relevant information to administrative staff, such as client admission and discharge and changes in public assistance cases to ensure the smooth functioning of the family and compliance with numerous regulatory agencies.
    • Perform other duties and special projects as required.
    • Facilitate effective communication with other staff within an interdisciplinary context, participate in and contribute to staff meetings and the development of policies and procedures.
    • Participate in training and staff-development activities to acquire and maintain both interpersonal and operational skills necessary for successful job performance.
    • Serve as a super user in the DHS CARES System to assist the social service staff in trouble-shooting problems in the system.
    *MINIMUM QUALIFICATIONS:
    * *Education: *Bachelor's degree in Social Work or related field required
    * *Experience: *At least two years of experience in a related field. Experience with multi-problem families desirable.
    * *Related Skills: *Minimum of one to two years' experience in social services, counseling, crisis intervention, family services, preventive services or housing services. Experience facilitating groups for homeless adults, specialized knowledge of substance abuse issues and treatment, and family dynamics treatment is necessary.
    * *Related Skills: *Excellent interpersonal and communication skills, both written and verbal. Bilingual language ability highly desirable. PC literate, with MS-Office.

    * *Other Requirements (including Physical Demands)

    • Travel required.
    • May be required to work long hours and a varied schedule.

    • Demonstrated knowledge of and proficiency in CARES.
    • Demonstrated experience working in shelter setting and/or with high-risk families, including families with documented histories mental health, substance abuse, Domestic Violence, child abuse/neglect.
    • Ability to work effectively with a wide range of community-based providers.
    • Working knowledge of Case Management theories and approaches.
    • Knowledge of community health services, social service support agencies and networks.
    • Familiarity with DHS/OTDA social service documentation policies and procedures.
    • Critical thinking, analytical and problem-solving skills as well as resourcefulness.
    • Demonstrated ability to be flexible, work independently as well as with a team.
    • Familiarity with housing vouchers (i.e., Section 8, FEPS, SEPS, LINC, FHEPS, SOTA, EOSD, City FHEPS).
    • Familiarity with SOI (Source of Income Discrimination) and Human Rights Law regulations.
    • Ability to assess needs and assist residents in developing housing plans for promotion of self-sufficiency and permanent housing.
    • Ability to travel in the boroughs of New York City and its adjacent counties as needed.
