_We are looking for a qualified and compassionate LPN to help with our ever-expanding census in the East Valley. This is a TRIAGE position and will be from 5pm to 1am Monday-Thursday. Work will be primarily on the west side of the valley. _

JOB SUMMARY:

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse.

Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help care for hospice patients.

QUALIFICATIONS:

Educational/Degree: Graduate from an accredited practical nurse or vocational nursing program.
Training/Licensure: Currently licensed as an LPN to practice in the state of Arizona.
Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program.
Must have a current valid driver's license, auto liability insurance and reliable transport

Experience:
Minimum 2 years of experience as a nurse, preferred.

Active patient contact within past three years, preferred

Hospice experience, preferred

Essential Functions:

Provides direct patient care as defined in State Nurse Practice Act.
Implements current nursing practice following a patient assessment and an approved plan of care initiated by the Registered Nurse.
Provide accurate and timely documentation of patient services to reflect the plan of care.
Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient.
Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical or emotional condition.
Participates in oncall system and is responsible for providing oncall coverage when unavailable for assigned duties.
Performs other duties as assigned by the Case Manager and/or Supervisor.

$ $32.00 per hour

No more than 32 per week
