JOB SUMMARY:
The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse.
Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help care for hospice patients.
QUALIFICATIONS:
- Educational/Degree: Graduate from an accredited practical nurse or vocational nursing program.
- Training/Licensure: Currently licensed as an LPN to practice in the state of Arizona.
- Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program.
- Must have a current valid driver's license, auto liability insurance and reliable transport
Experience:
Minimum 2 years of experience as a nurse, preferred.
Active patient contact within past three years, preferred
Hospice experience, preferred
Essential Functions:
- Provides direct patient care as defined in State Nurse Practice Act.
- Implements current nursing practice following a patient assessment and an approved plan of care initiated by the Registered Nurse.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient.
- Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical or emotional condition.
- Participates in oncall system and is responsible for providing oncall coverage when unavailable for assigned duties.
- Performs other duties as assigned by the Case Manager and/or Supervisor.

Pay:
$ $32.00 per hour
