Primary Care Physician

About the Job:
As a Primary Care Physician at Oak Street Health, your main role is to provide high-quality, value-based healthcare to local medicare patients at our neighborhood primary care centers.

Our doctors work in a single medical center with strong support teams, allowing them to focus on providing excellent care rather than just services.

We offer a unique care model designed to reward positive outcomes, minimize hospital admissions, promote preventive medicine, and ensure exceptional patient satisfaction.

We are seeking dedicated physicians, including recent graduates, who are passionate about delivering outstanding care to older adults.

Outstanding candidates may also have the opportunity to be considered for a Center Medical Director role.

Responsibilities:

Evaluate and diagnose patients at local clinics
Lead primary care treatments and services
Prescribe medications and treatments
Manage patient health records efficiently
Collaborate with leadership to achieve quality health goals
Supervise the center care team
Other assigned duties

Required Qualifications:

Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
Board Certification in Internal or Family Medicine
Active State Medical License and DEA License
Work authorization in the USA
Alignment with Oak Street Health values

Preferred Qualifications:

Fellowship in Geriatrics
Experience with Geriatric care
Knowledge of Value-Based Healthcare Models
Ability to work in a team environment
Experience in outpatient care
Bilingual skills a plus
