Role Description:

The primary goal of a Primary Care Physician at Oak Street Health is to deliver high-quality, personalized healthcare to local Medicare patient populations in our innovative network of neighborhood primary care centers. Our Primary Care Physicians work at a single medical center supported by extensive care teams to ensure a focus on quality care rather than service quantity. We are seeking physicians who are genuinely committed to providing exceptional care to older adults, including recent graduates.

Exceptional candidates may also be considered for a Center Medical Director role.

Responsibilities:

Assess and diagnose patients at local clinics
Oversee, direct, and provide primary care
Prescribe and manage pharmaceutical treatments
Maintain Patient Electronic Medical Record data efficiently
Collaborate with regional and central leadership to achieve health quality objectives
Supervise and guide the center care team in coordination with Practice Managers
Perform any other assigned duties

Required Qualifications:

Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Degree
Board Certification in Internal Medicine or Family Medicine (Or board eligible)
Active, non-probationary, unrestricted State License
Active DEA license
Authorization to work in the USA
Proficiency in Spanish and embodying the core values of Oak Street Health

Preferred Qualifications:

Fellowship training in Geriatrics
Experience practicing medicine with Geriatric populations
Experience in a Value-Based Healthcare Model
Collaborative work experience ensuring positive health outcomes
Experience in outpatient primary care settings
