The Primary Care Physician role at Oak Street Health involves providing high-quality healthcare to local Medicare patient populations across neighborhood primary care centers. Physicians work within a supportive care team environment, focusing on delivering exceptional care rather than a high volume of services. Qualified candidates could be considered for a Center Medical Director role.


Responsibilities:
- Assess and diagnose patients at local clinics
- Administer primary care and oversee treatments
- Prescribe medication
- Maintain Electronic Medical Records using canopy and greenway
- Collaborate with leadership to achieve health quality goals
- Manage the center care team with Practice Managers
- Other assigned duties


Required Qualifications:
- M.D. or D.O. Graduate
- Internal Medicine or Family Medicine Board Certification preferred
- Valid State License and DEA license
- Work authorization in the USA
- Fluent in Spanish
- Ability to embody the "Oaky" spirit


Preferred Qualifications:
- Geriatrics Fellowship training
- Experience with Geriatric populations
- Proficient in Value-Based Healthcare Model
- Collaborative work experience for positive health results
- Outpatient primary care experience
