- Follows-up with former PATH clients to confirm maintenance of housing and mental health services and to identify clients who need re-engagement.
- Assists with client engagement and outreach, including in person visits and calls to last known contacts.
- Provides on-site assistance with service delivery and coordination.
- Provides peer support services to PATH clients as needed, including providing connection to Recovery Center.
- Maintains outreach log.
- Provides other duties as assigned to support the PATH teams.
- Attends weekly PATH meeting as available.

Description
Join our mission to help individuals with mental illness and addictions to lead healthy and productive lives. GCBHS has an immediate opening for a PATH Peer Specialist to serve adults who are experiencing homelessness and connect them to mental health and substance use disorder services. Responsible for providing support services to PATH staff and clients. Maintains PATH charts and data. Triages client phone calls, assists with client engagement and housing stabilization, and follows up on client's success with housing.Why Work at GCBHS?
