    Description


    Care Coordinator in Phoenix The Care Coordinator is a key member of the Behavioral Health Team and serves as an advocate for patient by coordinating care to minimize the wait for services, effectively assess needs of patients, and increase engagement in therapy and integrated services.

    They directly support evolvedMD's Behavioral Health Managers (BHMs) by promoting effective engagement of patients referred for external support services.

    Location will be hybrid work-from-home as well as on-site at one or more primary care practices in the Phoenix, AZ area.

    We do reimburse for mileage greater than 20 miles per day. Compensation $ $24.00 per hour (DOE) Bilingual stipend offered for English/Spanish speaking $1,200 annual technology allowance 401(k) company match Professional development stipend Rewards We Get You Where You Want to Be

    About you Bachelor's degree in a clinical behavioral health field (i.e., counseling, social work), preferred Two years of experience in case management, care coordination and/or behavioral health Potential for competency in multiple Electronic Heath Record Systems Ability to manage one's own time efficiently Bilingual in Spanish is preferred, not required You are most likely a fit if You are most impressed by a company's culture and want to work with like-minded and like-passioned people.
