    POSITION TITLE: SFFC Case Planner/Family Specialist

    PROGRAM: Specialized Family Foster Care (SFFC)

    DEPARTMENT: Family Permanency Services

    REPORTS TO: SFFC Social Work Supervisor

    SFFC Program Description

    Catholic Guardian Services' Specialized Family Foster Care (SFFC) Program helps special needs children to thrive in a supportive family environment. Specialized Medical Foster Parents (i.e. resource parents) receive training and support from program staff to allow them to meet the medical or developmental needs of the children.

    The program provides family permanency services to children and families served within the program. Our program teams are interdisciplinary and are composed of Family Specialists/Case Planners, Social Work Supervisors, Directors of Socio Services, Nurse Care Managers, Pediatrician, Parent Advocates, Case Aides and Licensed Socio-Therapists.

    Our work includes the implementation of evidence-based models to address trauma, mental health and other challenges faced by children and families. We successfully provide wrap-around services and support during and after foster care placement by having direct access to Catholic Guardian Services' portfolio of social services and behavioral health programs. These programs include children's health home care management services, evidence-based preventive services programs, outpatient mental health services, career development and job readiness services for adolescent. Our Enhanced Family Foster Care Teams closely collaborate with these programs to ensure ongoing support to our children and families.

    Major Duties & Responsibilities:
    • Participates in training and consultation sessions to develop skills in evidence-based and evidence-informed models and ensures that direct reports effectively utilize these models and do so with fidelity.
    • Attends internal and external meetings as necessary. Including interdisciplinary team meetings.
    • Maintains a professional and collaborative approach while interacting with the Socio-Therapist Team.
    • Participates in family team meetings and utilize learned skills that demonstrates an ability to understand, assess and engage families, children and foster parents in the service plan process and create workable and realistic service plans.
    • Engage families and effectively delivery services for purposes of reunification, addressing issues which necessitated placement.
    • Participate and complete of all required and mandated trainings.
    • Perform other duties as needed.
    COMPLIANCE OVERSIGHT

    Case Planners are instrumental in making sure that our casework practice and service delivery are aligned with the agency's and ACS' performance expectations. As such the agency's expectation is that all foster care programs Case Planners adhere to the following:
    • Comply with all due dates set by your supervisor for the completion and submission of all FASPs, PHRs, PYA and CANS.
    • Comply with all National Youth Transitional Database (NYTD) entries in Connections. This includes updating education tabs, making sure that ILS service goals are listed in all FASPs (or Plan Amendment) for all eligible youths, and documenting tribal affiliation progress notes.
    • Ensuring that Family Team Conferences (SPRs) are scheduled and held by identified due dates. If a conference does not take place on the first scheduled date a rescheduled date must be immediately requested.
    • To prevent higher rates of missed and rescheduled conference Case Planners are expected to reach out to birth families, foster families and children to identify good/bad times to hold the conference.
    • Monthly submission of your Case Planners' Self-Reporting Contact Compliance Report. Due to your supervisor the last workday of the month. The report tracks all required contacts and the timely documentation of these contacts.
    • Timely documentation of all progress notes. As per agency policy progress notes are to be documented within five (5) days of the event/activity.
    • Documenting child placement status change in Connections via a Notice/Approval Progress Note and making sure that a Child Tracking Form is submitted to your supervisor within 24 hours of such status change (i.e. AWOL, trial discharge, final discharge, hospitalization, detention facilities, etc.).
    • Monthly submission of your FEDERAL CONTACTS (Case Planner-Foster Child face-to-face contact) calendars to your supervisor by the 4th Monday of the current month for the upcoming month.
    • Making a successful face-to-face contact with EVERY child on your caseload by the 15th of each month and bringing missed contacts to the attention of your supervisor if not achieved by required date.
    • Ensuring the timely completion of-
      • PYA Discharge Checklists
      • Sex trafficking screening in the Child Trafficking Database (CTDB)
      • MHCU hospital admission and discharge notifications to ACS
    • Making sure that you are prepared for and attend ALL court hearings on time. This includes preparing the Court Update Report before the court date, discussing the case with your supervisor before court, reaching out to the FCLS Attorney before the court hearing.
    • Attending and being prepared for all scheduled supervision sessions as scheduled by your supervisor, attending all unit meetings and program/site meetings
    QUALIFICATIONS
    • Bachelor's degree in Social Work or related field
    • Child welfare experience preferred
    • Bi-lingual (Spanish) a plus
    • Ability to work a flexible schedule, including periodic evenings and weekends, and ability to travel to community settings throughout NYC
    • NYS Driver's license preferred

    • Competence in assessing family assessments in a community-based setting, as well as, knowledge of the foster care system.
    • Must possess a deep interest and concern for the preservation of families and a thorough knowledge of all areas of ACS standards and regulations.
    • Demonstrated knowledge of best practices in the field of serving families and children involved with the child welfare system.
    • Proven professional skills, including meeting deadlines, ensuring compliance with agency policy and procedures, and overseeing complete and timely maintenance of agency records, in accordance with contractual requirements.
    • Excellent writing and communication skills.
