Job Summary

The Family Case Planner will work in a child welfare prevention program that utilizes the Solution Based Casework model to stabilize families who have children under the age of 18 that are at risk for placement outside their home.

Home visits are required.

Job Responsibility
The goal of preventive services is to stabilize and provide support so that children can remain in their homes. The program is located in the East Flatbush section of Brooklyn but covers CD 15, 17 and 18 (Sheepshead Bay, East Flatbush, Canarsie, Mill Basin, Bergen Beach)

Qualifications

You are a good fit if you have:
Minimum of Bachelor's degree with some child welfare experience.

Must be willing to do home visits.

Good verbal and written communication skills.

Ability to learn the SBC model to partner with families to establish Action Plans and assist them to reach their goals.

The goal of preventive services is to stabilize and provide support so that children can remain in their homes.
