The Asthma and Chronic Disease Program Coordinator will play an integral role in building and expanding population health work in the Department of Pediatrics.

The role will provide coordination and data analytics support for our chronic disease and population health efforts with a primary focus on the coordination of all elements of our robust Pediatric asthma program.

They will work under the supervision of the Division Director of Pediatric Pulmonary and Allergy, Dr.

Robyn Cohen; the Physician Champion within Pediatric Primary Care, and in close collaboration with primary care Nursing and Operations leadership and BMC population health.

As a key member of the Asthma Team for the Department of Pediatrics the coordinator will help lead multiple initiatives aimed at improving care of asthma patients at Boston Medical Center (BMC) and its affiliated neighborhood health centers, to reduce the burden of this disease for our patients.

These initiatives include using the BMC Pediatric Asthma Registry to identify patients with asthma for symptom screening at every possible opportunity, supporting the standardization of acute and chronic asthma care within Primary Care, continuing our comprehensive Pediatric Asthma Education Program, optimizing outreach and care coordination for our highest risk patients, utilizing the Pediatric asthma registry to analyze data, report outcomes, working with affiliated health centers and key stakeholders on disseminating guideline-based asthma QI, and presenting the results of these initiatives at the local and national level.

The Asthma and Chronic Disease Program Coordinator will be responsible for managing or contributing to most of these initiatives, and working towards expanding the asthma and chronic disease programs in BMC Pediatrics through program development, QI, analysis and research.


ESSENTIAL RESPONSBILITIES/DUTIES:


  • Collaborate with clinical leadership to support program coordination, data analytics and quality improvement initiatives in population health associated with the most common Pediatric chronic diseases, with a primary focus on asthma.
  • Streamline and standardize the Primary Care, Pulmonary/Allergy, ED, and inpatient efforts around asthma education and QI for patients and providers with an emphasis on guideline based care, reducing barriers to care, and optimizing opportunities for individualized family education.
  • Collaborate closely with nursing, operations leadership, and pharmacy to ensure and track asthma control assessments and medication adherence.
  • Develop and implement workflows for as well as provide patient and family
- centered asthma education across multiple care settings including inpatient units, during outpatient visits, and during remote Telemedicine encounters. This will be in close collaboration with BMC Pediatric Primary Care nursing colleagues. Identify opportunities for work flow changes to improve quality of care related to the overall health and wellness of our asthma patients.

  • Develop standardized educational materials for use throughout BMC Pediatrics.
  • Work with IT to maintain and optimize asthma registry and incorporate validated assessment tools and asthma education materials into EPIC.
  • Maintain the Asthma Education Program database and create reports to monitor productivity and impact.
  • Collaborate with Implementation and Evaluation team members from the Center for the Urban Child and Health Family to optimize program development, rollout, and evaluation of new and existing initiatives within the Asthma Program.
  • Maintain collaborative partnerships with school nurses and schoolbased health clinics regarding patients' asthma care plans.
  • Serve as the BMC Pediatrics liaison with community partners (i.e., the Boston Public Health Commission) to improve access to and uptake of homebased asthma services.
  • Submit and manage IRB protocols related to asthma outcomes research.
  • Analyze data from the asthma studies and initiatives at BMC/BUSM as well as national studies.
  • Present outcomes of asthma programs and studies at the institutional and national level (conferences).
  • Remain current on literature re; asthma management, education, program planning, and evaluation.
  • Represent the Program and BMC mission on internal and external committees, task forces, etc. as appropriate.
  • Must adhere to all of BMC's RESPECT behavioral standards

EDUCATION:

Bachelor's required. Master's preferred in public health, health administration or a related field.


EXPERIENCE:


Required:
Minimum of 3-5 years' experience working with individuals with chronic conditions and/or their families.

Desired: 5 years of program development and research experience in a clinical setting desired, 2 years with Master's degree

Quality improvement and research experience a plus
