Job DescriptionPosition Summary:
Project R.I.T.E is a SAMHSA funded program in support of the LGBTQAI+ community in navigating health care and social supports including HIV/HCV Prevention and Care. Project R.I.T.E focuses on aiding LGBTQIA+ communities color with increased risk for HIV/HCV, living with HIV or have a co-occurring mental health disorder. The Project R.I.T.E Case Manager is responsible for providing HIV counseling and testing on and off site; coordination of care for enrolled patients based on identified needs, establishing professional linkages with identified HIV/AIDS and health and social support services providers in the targeted project communities. Moreover, the Case Manager will work closely with the Project R.I.T.E team to support ongoing referrals and follow up to reinforce HIV/HCV prevention initiatives at The Joseph P. Addabbo Family Health Center, In
This is an early career level position that is ideal for professionals who have one to five years of experience working in HIV prevention, behavioral health, social services, and other community-based advocacy.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Lead Project R.I.T. E's enrollment, retention, and engagement efforts for qualifying persons.
2. Coordinating linkage to care for clients seeking treatment adherence support, Housing, entitlements, risk reduction education and health insurance.
3. Develop and facilitate educational seminars on HIV/ AIDS/ STD prevention and access to services.
4. Conduct sexual and social history screenings for linkage to appropriate care/services .
5. Engage patients in health education and support towards optimizing health outcomes
6. Assist enrolled patients in accessing various community health, nutrition, and social support services.
7. Ensure appropriate follow-up for client referrals to external resources such as substance use treatment, mental health services, restorative justice, legal services, and nutrition support.
8. Perform special projects and any other tasks as assigned.
9. Support Outreach Specialist with planning and coordinating educational outreach events
10. Foster relationships community partners to streamline referrals and linkages to social services.
11. Conduct clinic rotations and address HIV testing, linkage, and retention service gaps.
12. Facilitate Training and Technical Assistance to Clinical and Non-Clinical staff on grant processes and procedures.
13. Work closely with Care Teams across the JPA network to provide high quality comprehensive care to patients throughout Addabbo clinics.
14. Performs other job-related duties as assigned.
Minimum Qualifications
1. Bachelor's Degree in Human Services, Social Work or related field preferred, but equivalent community work experience and/or other certifications will be considered for exceptional candidates.
2. Members of the LGBTQIA+ community strongly encouraged to apply
