Job Summary:

The
LPN in Long Term Care (LTC) plays a crucial role in providing high-quality medical care and support to patients in long-term care facilities.

This position requires strong clinical skills, empathy, and dedication to ensure the well-being of residents.

The
LPN collaborates with interdisciplinary teams to establish care plans, monitor patient health, administer medications, and ensure a safe and comfortable environment.

Responsibilities and Duties:

Evaluate patient conditions and collaborate with the healthcare team to develop individualized care plans.
Administer medications and treatments according to physician orders, monitoring and documenting their effectiveness.
Observe and assess patients' health, including vital signs, symptoms, and overall well-being, reporting any changes to the supervising nurse or physician.
Assist patients with activities of daily living, including bathing, dressing, toileting, and mobility, ensuring their comfort and dignity.
Collaborate with other healthcare professionals, such as therapists and dietitians, to ensure patients receive comprehensive care.
Implement infection control measures to maintain a safe and healthy environment for patients and staff.
Document accurate and timely medical records, including patient assessments, interventions, and outcomes.
Educate patients and their families on disease management, medication administration, and self-care techniques.
Respond to emergencies and provide immediate medical assistance as necessary.
Collaborate in interdisciplinary meetings to contribute to patients' care plans and address their changing needs.
Participate in ongoing professional development to enhance knowledge and skills in long-term care practices.
Maintain effective communication with patients, families, and coworkers to ensure continuity of care and patient satisfaction.
Adhere to ethical and legal standards in the delivery of care and maintain confidentiality of patient information.
Comply with all facility policies, regulations, and procedures to maintain a safe and compliant healthcare setting.
Advocate for patient rights, safety, and quality care within the facility.

Qualifications and Skills:

Valid
LPN license in the state of employment.
Completion of an accredited
LPN program.
Previous experience in long-term care or geriatric nursing is preferred.
Knowledge of nursing principles, practices, and standards related to long-term care.
Strong clinical assessment skills and the ability to make sound decisions.
Excellent communication and interpersonal skills to interact effectively with patients, families, and members of the healthcare team.
Proficient in electronic health record (EHR) systems and documentation.
Ability to work efficiently in a fast-paced environment and prioritize tasks.
Compassionate and empathetic approach towards residents and their families.
Ability to work collaboratively in interdisciplinary teams.
Basic computer proficiency in MS Office suite.
