Job Summary:


The
LPN in Long Term Care (LTC) plays a crucial role in providing high-quality medical care and support to patients in long-term care facilities.

This position requires strong clinical skills, empathy, and dedication to ensure the well-being of residents.

The
LPN collaborates with interdisciplinary teams to establish care plans, monitor patient health, administer medications, and ensure a safe and comfortable environment.


Responsibilities and Duties:

  • Evaluate patient conditions and collaborate with the healthcare team to develop individualized care plans.
  • Administer medications and treatments according to physician orders, monitoring and documenting their effectiveness.
  • Observe and assess patients' health, including vital signs, symptoms, and overall well-being, reporting any changes to the supervising nurse or physician.
  • Assist patients with activities of daily living, including bathing, dressing, toileting, and mobility, ensuring their comfort and dignity.
  • Collaborate with other healthcare professionals, such as therapists and dietitians, to ensure patients receive comprehensive care.
  • Implement infection control measures to maintain a safe and healthy environment for patients and staff.
  • Document accurate and timely medical records, including patient assessments, interventions, and outcomes.
  • Educate patients and their families on disease management, medication administration, and self-care techniques.
  • Respond to emergencies and provide immediate medical assistance as necessary.
  • Collaborate in interdisciplinary meetings to contribute to patients' care plans and address their changing needs.
  • Participate in ongoing professional development to enhance knowledge and skills in long-term care practices.
  • Maintain effective communication with patients, families, and coworkers to ensure continuity of care and patient satisfaction.
  • Adhere to ethical and legal standards in the delivery of care and maintain confidentiality of patient information.
  • Comply with all facility policies, regulations, and procedures to maintain a safe and compliant healthcare setting.
  • Advocate for patient rights, safety, and quality care within the facility.

Qualifications and Skills:

  • Valid
    LPN license in the state of employment.
  • Completion of an accredited
    LPN program.
  • Previous experience in long-term care or geriatric nursing is preferred.
  • Knowledge of nursing principles, practices, and standards related to long-term care.
  • Strong clinical assessment skills and the ability to make sound decisions.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and members of the healthcare team.
  • Proficient in electronic health record (EHR) systems and documentation.
  • Ability to work efficiently in a fast-paced environment and prioritize tasks.
  • Compassionate and empathetic approach towards residents and their families.
  • Ability to work collaboratively in interdisciplinary teams.
  • Basic computer proficiency in MS Office suite.
