Job Summary:
The
LPN in Long Term Care (LTC) plays a crucial role in providing high-quality medical care and support to patients in long-term care facilities.
The
LPN collaborates with interdisciplinary teams to establish care plans, monitor patient health, administer medications, and ensure a safe and comfortable environment.
Responsibilities and Duties:
- Evaluate patient conditions and collaborate with the healthcare team to develop individualized care plans.
- Administer medications and treatments according to physician orders, monitoring and documenting their effectiveness.
- Observe and assess patients' health, including vital signs, symptoms, and overall well-being, reporting any changes to the supervising nurse or physician.
- Assist patients with activities of daily living, including bathing, dressing, toileting, and mobility, ensuring their comfort and dignity.
- Collaborate with other healthcare professionals, such as therapists and dietitians, to ensure patients receive comprehensive care.
- Implement infection control measures to maintain a safe and healthy environment for patients and staff.
- Document accurate and timely medical records, including patient assessments, interventions, and outcomes.
- Educate patients and their families on disease management, medication administration, and self-care techniques.
- Respond to emergencies and provide immediate medical assistance as necessary.
- Collaborate in interdisciplinary meetings to contribute to patients' care plans and address their changing needs.
- Participate in ongoing professional development to enhance knowledge and skills in long-term care practices.
- Maintain effective communication with patients, families, and coworkers to ensure continuity of care and patient satisfaction.
- Adhere to ethical and legal standards in the delivery of care and maintain confidentiality of patient information.
- Comply with all facility policies, regulations, and procedures to maintain a safe and compliant healthcare setting.
- Advocate for patient rights, safety, and quality care within the facility.
Qualifications and Skills:
- Valid
LPN license in the state of employment. - Completion of an accredited
LPN program. - Previous experience in long-term care or geriatric nursing is preferred.
- Knowledge of nursing principles, practices, and standards related to long-term care.
- Strong clinical assessment skills and the ability to make sound decisions.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and members of the healthcare team.
- Proficient in electronic health record (EHR) systems and documentation.
- Ability to work efficiently in a fast-paced environment and prioritize tasks.
- Compassionate and empathetic approach towards residents and their families.
- Ability to work collaboratively in interdisciplinary teams.
- Basic computer proficiency in MS Office suite.
