    JOB TITLE: CASE MANAGER I - OUTREACH

    JOB DETAILS:

    Works in the community and assists individuals experiencing homelessness where they are, including the streets, under bridges, and in abandoned houses. The street outreach team engages, assesses, treats (medically and mentally) and secures permanent supportive housing placement for persons who are chronically homeless in Miami-Dade County.

    Salary

    $40,000 -42,000.00/annual

    Specialties

    Education: Bachelor's degree preferred in Social Work, Psychology, or related field; any appropriate combination of education, experience and/or certification may be considered.

    Experience: Minimum 2 years of experience in a similar job role preferred.

    Language: Proficiency in oral/written English. Bilingual (i.e., Spanish) preferred; additional languages a plus (i.e., Creole).

    Skills: Proficiency in computer (Word, Excel, other data collecting programs).

    Requirements

    Successful completion of Toxicology test, L2 and local background screening.

    Valid FL Driver License with clean driving record.
    Legally authorized to work in the United States.

    ABOUT CAMILLUS: For more than 62 years, Camillus House has provided a variety of programs to address the needs of individuals who are mentally ill and suffer from substance abuse disorders, with the end goal of helping them achieve housing stability. One of the largest and most comprehensive non-profit providers of housing and services for the homeless in Miami, Camillus offers those served compassionate and professional care through our DCF-licensed and CARF-Accredited behavioral health programs and specialized outreach teams, and our staff a rewarding career that makes a difference.

    QUALIFICATIONS:

    • Be comfortable working directly with highly vulnerable populations in an outpatient basis as part of a multidisciplinary street outreach program.
    • Have competency in MS Office/Outlook and other data entry programs.
    • Works with referral sources and community partners to identify applicants through team meetings, outreach, and referrals.
    • Completes interviews with individuals to gather information to complete SSI/SSDI applications.
    • Gathers medical records and other information to complete SSI/SSDI applications.
    • Writes SOAR Medical Summary Reports for individual applications.
    • Accompanies individuals to appointments at the Social Security Administration
    • Coordinates visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application.
    • Coordinates case management services with partners and help with providing case management services to individuals when needed.
    • Assists the team with administrative tasks as needed.
    • Tracks all SOAR applications through completion, including appeals as needed.
    • Ensures data entry into OAT, the national SOAR database.
    • Documents client engagement through case notes according to established policies and procedure
    • Participates in staff organizational meetings and treatment planning review meetings.
    • Drives clients to appointments, as needed.
    • Other duties as assigned.
