Overview:
Responsibilities:

Client Assessment: Conduct comprehensive assessments of clients' needs and develop personalized care plans in collaboration with clients, their families, and healthcare professionals.

Coordination: Coordinate home care services, including scheduling caregivers, arranging transportation, and managing medical appointments.

Communication: Serve as a liaison between clients, caregivers, healthcare providers, and community resources to ensure continuity of care.

Advocacy: Advocate for clients' rights and access to appropriate services, addressing any barriers or challenges they may encounter.

Education: Provide clients and their families with information on available resources, disease management, and self-care techniques.

Documentation: Maintain accurate and up-to-date records of client assessments, care plans, and service delivery in compliance with regulatory standards.

Crisis Intervention: Respond to emergencies and crisis situations promptly, providing support and guidance to clients and their families.

Quality Assurance: Monitor the quality of care provided to clients, conducting regular evaluations and implementing improvements as needed.

Training: Provide training and support to caregivers to ensure they have the necessary skills and knowledge to deliver high-quality care.

Cultural Competence: Demonstrate sensitivity to cultural differences and respect for clients' diverse backgrounds and beliefs.
Qualifications:
- Bachelor's degree in Social Work, Nursing, Psychology, or related field (preferred).
- Previous experience in home care coordination, case management, or a related role.
- Fluency in English and Spanish.
- Strong communication, interpersonal, and organizational skills.
- Knowledge of healthcare systems, community resources, and government programs.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate, empathetic, and nonjudgmental approach to client care.
- Proficiency in computer skills and electronic health record systems.
