Your tasksResponsibility for the participation and overall planning process
in accordance with §§ 117 ff./§§ 19 ff. SGB IX for people with a significant disability
Identifying and formulating the need for integration assistance as well as
determining the types of services (§ 118 SGB IX)
Selecting the service provider together with the affected individuals while
respecting their right to choose
Creating participation and overall plans and monitoring the achievement of goals
Coordinating and evaluating the interaction of services
Your profile
Completed degree in social work, social pedagogy, or special education
(FH/BA diploma or bachelor's degree)
or equivalent qualification
Experience in working with people with disabilities
Knowledge of significant medical conditions and their effects
Willingness to work in an interdisciplinary team
Knowledge of local structures or willingness to quickly familiarize oneself with them
Teamwork skills, willingness to work independently
Knowledge of case management
Overall planning process/participation process
Empathy, communication skills, negotiation skills, assertiveness, and knowledge of
methodical conversation techniques
