Description
A Community Wellness Advocate (CWA) is a trusted member of the community who helps high-risk patients maintain stable health and wellness through integrating hospital, home-based, and community-based services. CWAs provide advocacy and case management services, develop interdisciplinary care plans based on identified patient needs, facilitate access to social service resources, monitor patient progress, and problem-solve with patients to enhance access to concrete supports.
- Initiates face-to-face contact with eligible patients to describe the role and explain participation benefits.
- Schedules and completes initial screenings, care plans, and follow-up visits for enrolled patients.
- Teaches key educational messages using culturally appropriate strategies in various settings.
- Documents all activities in the patient's record and care management system.
- Participates in community outreach activities and presentations, collaborates with staff, and sets goals for patient care.
- Connects patients with community resources and programs for disease self-management.
- Consults with healthcare teams regarding complex patient situations and continuously develops optimal care plans.
- Monitors participant progress, documents assessments, and updates patient data.
- Provides advocacy, education, and assists patients in accessing community-based programs and services.
- Establishes trusting relationships with patients and their families and participates in training activities.
- Develops strong relationships with the community and community resources.

ESSENTIAL RESPONSIBILITIES / DUTIES:
- Initiates face-to-face contact with eligible patients to describe role, explain participation benefits, and begin screening process.
- Schedules and completes initial hospital, clinic, or community-based visits for enrolled patients.
- Teaches educational messages using culturally appropriate strategies.
- Documents all activities in the patient's record.
- Participates in community outreach and presentations.
- Works with patients and providers to set goals for patient's care.
- Connects clients with community services and programs for disease self-management.
- Presents patients at case review meetings.
- Consults with healthcare teams regarding complex patient situations.
- Functions within an interdisciplinary team, connecting patients with resources.
- Records and monitors participant progress toward goals.
- Documents assessments and updates in the system; prepares reports as needed.
- Provides advocacy and successful warm hand-offs in accessing programs.
- Assists patients in overcoming barriers with concrete supports.
- Coordinates with long-term services and supports.
- Provide outreach, motivational interviewing, and resource connection to help patients.
- Establishes relationships with patients and their families.
- Participates in training activities and program meetings.
- Develops relationships with the community and resources.
NOTE: The CWA will not provide hands-on care or home health services.
EDUCATION: HS Diploma with community experiences or Bachelor's degree
Driver's license required/reliable transportation required
EXPERIENCE: Minimum of 2 years in a health-related field
