- Collaborate with the PCP Consultant to identify gaps in treatment for clients on the high utilizer report
- Outreach clients on the high utilizer report to develop plans to address frequent hospitalizations
- Remain a resource for clients desiring smoking cessation support
- Assist in outreach and enrollment for the HCH team
- Provide clerical support to the HCH team as needed
- Implement required DMH Net Health technology programs such as Cyberaccess and CareManager
- Assist with tracking hospitalizations and communication with the treatment team
- Degree in Nursing (LPN or RN), Counseling, Social Work or other social services field with at least 2 years of experience
- Preferred knowledge of community resources
- Knowledge of chronic health conditions in both youth and adult populations
- Excellent written and verbal communication skills
- Knowledge of mental health conditions in youth and adult populations
- Ability to make decisions based on critical thinking
