    Department Overview


    The Oregon Office on Disability and Health (OODH) is a CDC-funded program to promote health equity and well-being of people with disabilities.

    Function/Duties of Position


    This position will be responsible for linking adults with intellectual and developmental disabilities (IDD) to needed preventive health care and health promoting services, and for building community relationships to support those linkages.


    Primary duties of the position include:

    • Foster relationships and contribute to new partnerships with community organizations and leaders, particularly in Latino and Native American communities, to respond to needs within the context of the project. Collaborate with communities to develop culturally-appropriate tools that support linking to preventive health care, health promotion, and other services/
    • Enroll participants, provide them with health education, link them to resources, and conduct follow-up assessments.
    • Collect evaluation data from project participants.
    • Contribute to design and content of both an internal directory of health care and health promoting resources and a health resource navigation guide for use by case management entities and other organizations, with a focus on integrating community-based perspectives of navigating care.
    • Note: This will be a one year position with possibility of extension dependent of funding.
    Required Qualifications


    • Bachelor's in relevant field OR Associates AND 3 years of relevant experience OR 4 years of relevant experience OR Equivalent combination of training and experience.
    • Experience (personal or professional) with adults with intellectual and developmental disabilities.
    • Strong community networking and interpersonal skills
    • Commitment to health equity
    • Understanding of healthcare systems and barriers to care
    • Strong written and oral communication skills
    • High level of organization
    Preferred Qualifications


    • Community Health Worker or Certified Prevention Specialist license.
    • Experience in case management, care coordination, traditional or community health work, or complex systems navigation.
    • Bilingual in Spanish strongly preferred
    • Experience (personal or professional) with Latino or Native American communities.
