SM The Care Coordinator role is a critical member of the Landmark Interdisciplinary Team. This role reports to Care Coordinator Supervisor, or Senior Operations Manager.

    Responsibilities are to ensure a positive patient experience with Landmark, as well as to build good relationships with local provider groups to ensure that Landmark is a reliable and trusted partner in patient care.

    This position is full-time, Monday
    • Friday.
    Employees are required to work our normal business hours of 8:30am - 5:00pm EST. It may be necessary, given the business need, to work occasional overtime or weekends. We offer up to 3 weeks of training. The hours during training will be 8:30am to 5:00pm EST, Monday
    • Friday.
    Training will be conducted virtually from your home. _All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter Policy.


    _ Primary Responsibilities:
    Regular contact with provider groups to collect clinical data. Support patients in scheduling home visits. Ensure that Landmark has all documentation available regarding patient conditions. Support Landmark providers and Nurse Care Managers with operational and clinical tasks. Handle escalated patient issues, ensuring the appropriate resources are involved. Follow triage protocol when needed. Other duties may be assigned.

    Required Qualifications:
    High School Diploma / GED OR equivalent work experience Basic Proficiency in Microsoft office applications - Microsoft Excel (open, enter, edit, save data), Microsoft Word (opening, reading, saving files), and Outlook (email, calendar)


    Experience handling customer service calls (inbound and outbound) Must be 18 years of age OR older Ability to work our normal business hours of 8:30am to 5:00pm EST, Monday - Friday Preferred Qualifications: Experience as a Medical Assistant, Pharmacy Tech, or other clinical or administrative role in a health care or office setting (i.e.

    handling electronic medical records, charting, scheduling tasks)

    The hourly range for California / Colorado / Connecticut / Hawaii / Nevada / New York / New Jersey / Washington / Rhode Island residents is $ $28.27 per hour.

    Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
