Care Coordinator - Hartford, United States - Hispanic Health Council
Description
The Care Coordinator serves as part of the Maternal and Child Health (MIOP) unit. Under supervision, the Care Coordinator will implement Home Visiting practices for clients and their children.Follows up with referrals, providing comprehensive services to clients during pregnancy, post-partum, during and through pregnancy stages, and infant support, up and through the 18th month of the child.
Establishes outreach activities to recruit clients. Conducts community canvassing, tabling events, and door-to-door outreach services. Ensures quality of care through advocacy, health education, and social and emotional support. Ensures thorough and accurate documentation of client care, registering data into program database. Participates in quality assurance, client satisfaction, and case management. Provides in-person and virtual home visits and educational sessions. Maintains detailed home visiting data and spreadsheet, preparing programmatical reports. Reports on cases and findings. Performs related work as required.The hours of work are 35 per week. Required to travel to site locations.
This is a grant funded position. Position is contingent on grant funding.
Knowledge, Skills, and Abilities
- Knowledge of program management principles
- Knowledge of technical platforms (
Microsoft Suite:
Excel, Teams, etc.)
- Ability to plan and implement program initiatives
- Ability to work independently
- Ability to display professionalism through marketing and outreach efforts
- Ability to work flexible work hours including prescheduled nights and weekends
- Strong communication and organizational skills
Qualifications
High School Diploma or GED and one (1) years of experience providing education, direct care services or closely related field to a diverse population.
Preferred Qualifications:
- Associate's or Bachelor's Degree in Social Service, Early Childhood Education or related field
- Experience with at risk population
- Bilingual
- Spanish/English
