Description

The Hampden County Sheriff's Office is seeking a Care Coordinator to facilitate educational and treatment groups, as well as aftercare planning, at our Main Institution in Ludlow, Massachusetts.

The Care Coordinator will establish and maintain counseling relationships with clients, manage relevant information, and submit necessary reports to ensure the safe, effective, and efficient operation of the program, in line with prescribed policies and procedures.


Work Schedule:
Monday-Friday; 9 a.m. - 5 p.m.

REQUIREMENTS:


  • At least two (2) years of clinical experience in the field of substance addiction treatment.
  • High School Diploma or equivalent required; CADC and LADC preferred.
  • Ability to establish and facilitate a therapeutic rapport with client population.
  • Strong computer skills with working knowledge of Microsoft Office 2010.
