    Description

    A hospital system in the lower Hudson Valley is currently seeing a Director of Case Management to join their organization.

    About the opportunity:

    • Schedule: Monday to Friday
    • Hours: Standard business hours
    • Specialty: Case Management and Utilization Review
    • Setting: Acute Care

    Responsibilities:

    The Director of Case Management (RN) will:

    • Establish and coordinate utilization and discharge planning functions for the facility
    • Identify and implement processes that support and promote efficient patient throughput across the healthcare continuum
    • Coordinate and facilitates multiple resources, internal and external, into a single approach to care coordination for each patient assigned case management services
    • Oversee social work and case management and utilization review team
    • Participate in quality metrics and data review analysis
    • Other duties as needed

    Qualifications:

    • NYS RN
    • Masters in Nursing
    • 10-15 years experienced as an RN
    • Strong leadership experience overseeing

    Desired Skills:

    • PRI certification
    • CCM certification
