- Provide advanced and complex medically necessary clinical services, including but not limited to, assessment, individual, family & group therapy & crisis intervention services to a targeted population of Adult & Youth patients and their families. Services may be provided through a telehealth platform, in the clinic, in the patient's home, or other community setting.
- Responsible for case management services for assigned caseload. These services include: referrals, advocacy, interagency staffings, and coordinating care between agencies/service providers as appropriate.
- Formulate recovery-based plans of care with person centered goals and objectives. Conduct timely reviews and maintain accurate clinical documentation.
- Attend and participate in Treatment Team meetings/staffings, including staffing with MD at CCMHC. Pursue continuing education and training. Maintain positive professional relationships with MHC staff, and community agencies, as well as patients and their families. Work to overcome the stigma of mental illness and mental health services by providing education, training and support services to the community at large.

Job Responsibilities
With our Agency's mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina.
This position is located at the Department of Mental Health, Catawba Community Mental Health Center (CCMHC), Chester Clinic, 524 Doctor's Court, Chester, SC 29706.
Under the limited supervision of the Chester County Clinic Director, the employee will provide therapeutic and emergency services to Adult and Youth Patients and their families. Will also ensure frequent interaction with other community agencies and maintain positive professional relationships.
Responsibilities for this Position Title include but are not limited to:
This position MAY be eligible for SCDMH Student Loan Repayment to cover some of the cost of your outstanding loan(s) balance associated with the completion of a healthcare degree that is relevant to the appliable position.
Minimum and Additional Requirements
State Minimum Requirements:
A Master's degree in guidance, counseling, the social sciences, or a related field.
Agency (Facility) Minimum Requirements:
Master's degree in guidance, counseling, or the social sciences two years of experience in a clinical counseling program preferred.

Preferred Qualifications
Bilingual abilities in English and Spanish (or another language) are a plus.
