    Location: Brooklyn, NY 11238

    Community Oriented Recovery and Empowerment (CORE) Services are person-centered, recovery-oriented, mobile behavioral health supports intended to build skills and self-efficacy that promote and facilitate community participation and independence. CORE services includes HARP enrollees and HARP-eligible HIV/SNP enrollees who would benefit from CORE Services in the pursuit of valued recovery goals.

    SCOPE OF ROLE: Supporting independence and community inclusion, S:US' Treatment Services division promotes person-centered services that focus on recovery. We apply evidence-based practice, data-driven accountability, compassion and dedication to our work.     ESSENTIAL DUTIES & RESPONSIBILITIES:

    • Work within a crisis driven environment with a knowledge base of substance use and addiction, domestic violence, trauma, cultural humility and mental health conditions.
    • Attend ongoing professional development by the individual agency program, MCTAC, CTAC, and OMH and DOHMH.
    • Individual Psychoeducation and Skill Building: Interventions designed to develop or improve members.
    • Personal coping strategies, prevent crises, and promote recovery.
    • Provide counseling to help the individual choose, get, and keep valued life roles.

    • Provide rehabilitation Counseling with Individualized goal-driven interventions that facilitate, promote, and improve functioning in living, working, learning, and social environments. (Educational & Vocational)
    • Provide counseling to help the individual choose, get, and keep valued life roles.
    • Data monitoring of your billable and non-billable sessions.
    • Work from a strength based, member centered perspective and identify and fortify the strengths that each person has.
    • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being.
    • Provide clear written reports that capture key findings to include provider areas of strength, and recommendations for improvement.
    • Document member medical and psychological histories, educational/vocational histories, service plans, or outcomes.
    • Complete other tasks as deemed necessary by CORE Clinical Supervisor/ CORE Team Leader.
    • Ability to work a flexible schedule when needed which may consist of weekend or evening hours.
    • Additional tasks as assigned.

    REQUIRED EDUCATION AND EXPERIENCE

    • BA required in human services
    • Effective written and oral communication skills.

    PREFERRED QUALIFICATIONS & SKILLS

    • Knowledge of guidelines, policies and regulations of OMH, OASAS, and CORE Services
    • Expertise in strengths-based, solution-focused, trauma informed care, cultural humility, and person-centered care
    • Ability to work independently and as part of a multi-disciplinary team, to assist individuals of diverse ethnicities, backgrounds, ages, and sexual preferences.
