    Description

    Job Responsibilities

    :
  • Providing ongoing case management; coordinating behavioral health services, medical care, education, placement, finances, and socialization services to children and families with intense/complex needs.
  • Collaborating with the family and team members to create support plans that accurately capture the strengths, needs and culture of the family.
  • Facilitating CFT meetings; establishing an agenda, setting and maintaining a strength based meeting environment, staying within timeframes, giving all team participants the opportunity to contribute, holding people accountable for their commitments and the ability to assist a team in developing a consensus for children and families.
  • Assisting young people and families in securing needed community resources and linkages to other services. Helping families to identify natural and informal supports.
  • Service delivery can be inclusive of skills training and family support.
  • Interacting professionally with other stakeholders: Division of Child Safety (DCS), Division of Developmental Disabilities (DDD), juvenile probation, Direct Service Providers (DSP), etc.
  • Performing other duties related to the position.

    • This position requires afternoon, evening, and weekend work. Adaptability in scheduling, driving throughout the county and working within the homes and communities of the families we support is required.

    Education/Experience Needed:

  • High school diploma or GED .
  • Must be BHT with two (2) years of related behavioral health work experience, or a related bachelor's degree (preferred but not required).

    • Requirements:

  • Must be 21 years or older
  • Valid Arizona Driver's License with an acceptable 39 month driving record
  • Reliable transportation, including proof of Insurance
  • Candidates are required to submit a negative drug test
  • Obtain an Arizona DPS Level One Fingerprint Clearance Card
  • Complete Adult, Child, and Infant CPR and First Aid training
