The Assessment Coordinator (RN) makes assessments for plans and advocates for the residents of the community facility so they can reach their highest practical level of physical, mental and psychosocial well-being.
The Assessment Coordinator (RN) will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community.

Attends and participates in facility meetings and in-service training programs as assignedInitiates emergency measures according to the community facility policy and within standards of nursing practiceMaintains all documentation as required by Federal and State regulations, and the community facility.

Responsible for pre-admission assessment of referrals, coordinates admissions and discharge planning for all the residentsThe Nurse Admissions Coordinator coordinates and directs the care plan meetings.
The Nurse Admissions Coordinator provides on-going education of resident's rights, abuse and neglect prevention to staff. This position must be willing to work the floor as needed as well.Education, Experience and Requirements:
Associate or Bachelor's Degree in Nursing is strongly preferred. Minimum one years' experience in Assisted Living, Alzheimer's, or Memory Loss care preferred. Experience with Microsoft Word and Excel preferred.
Licensed in state of NJ and be responsible for maintaining professional licensure and CEU requirements.
