The Role:
As an Assistant Case Manager you will provide direct support to the individuals we serve in our Day Habilitation Programs.
Through the development and maintenance of positive relationships with those we serve, you will provide guidance, care coordination and physical assistance when necessary to achieve a person's individual goals.
You will motivate the people we serve to be empowered in their programs and will have the ability to make a real difference in the lives of people with intellectual and developmental disabilities.

What We're Looking For:
We're searching for a detailed and enthusiastic individual who will bring their personality, an independent work ethic, a strong belief in supporting the lives of those we serve with strong teamwork skills to join us as an Assistant Case Manager in our Day Habilitation Programs.
Are you someone who pays attention to the details, stays organized in a sometimes fast-paced environment, and can exercise good judgment in challenging situations? If yes, you might be the perfect fit for our team.
What You'll Do:
- Provide individual and small group instruction in selfcare, activities of daily, living, communication, social and motor skills.
- Provide direct care for ADLs and group activities, including but not limited to toileting, changing and feeding. Lift and transfer individuals as appropriate to their needs.
- Participate in scheduled supervision sessions with Assistant Director.
- Participate in team meetings regarding general program development, including curriculum development and schedules.
- Communicate respectfully and proactively with families, residential staff and all member of participants teams.
- Attend in service trainings as required.
- Comply with all applicable State and Federal regulations, agency policy and procedures, and personnel practices.
- Protect the health and safety of all participants and staff.
- Excellent interpersonal skills.
- Ability to work in a team environment.
- Perform other duties as requested by supervisor.

Education and experience:
- High School Diploma, experience in Human Services, healthcare support or education, with ability to demonstrate effective oral and written communication skills.
- Valid driver's license & personal vehicle required.
Pay:
Up to $31,560.00 per year
